Substance Use Disorder Counseling
Outpatient counseling, Substance Abuse/AODA prevention, intervention, and assessments for individuals and families.
(Including HOPE: Heroin and Opiate Prevention Education Grant) Our Recovery Program serves patients with Substance Use Disorder; providing outpatient (individual and group) counseling, medication assisted treatment, and abstinence based treatment as well as intensive case management and community supports. In 2015 we expanded the program with the help of the H.O.P.E. legislation and grant developed to respond to the growing concern around the heroin and opioid epidemic in Wisconsin. Psychiatric Medication Management
Outpatient psychiatric diagnostic evaluations, medication management, and case consultation for both adults and youth.
Patient Support Services
Because patients have needs that go beyond traditional health care, our patient resource services offer a variety of tools that can help people in need. Assistance with enrolling in benefits programs, transportation, foodshare, referrals and care coordination.
